Attuali orientamenti nella diagnosi e nel trattamento delle aderenze placentari patologiche: la nostra esperienza
2010
The abnormal adherence of the placenta is a disease characterized by an anomalous miometrial infiltration of Chorionic villi. This event blocks the regular detachment of placenta in post partum and sometimes a severe bleeding that causes the urgency of a caesarean section. In the past years the incidence of this pathology has increased to about 1 in 500/600 deliveries. The cause of abnormal adherence of placenta is unknown but there are some risk factors such as prior caesarean sections, women older than 35 years of age and placenta previa. The early diagnosis is very important and it can be done by Eco-Color-doppler (eventually associated with 3d technique) and Magnetic Resonance. Objectiv…
Morbidly adherent placenta: evaluation of ultrasound diagnostic criteria and differentiation of placenta accreta from percreta
2013
Objectives To evaluate the diagnostic accuracy of two-dimensional (2D) gray-scale and color Doppler and three-dimensional (3D) power Doppler sonographic criteria for morbidly adherent placenta (MAP), and to identify criteria to help distinguish placenta accreta from placenta percreta. Methods We enrolled 187 patients with placenta previa and history of uterine surgery and performed transabdominal and transvaginal ultrasound examination for early detection of MAP. With 2D gray-scale transabdominal and transvaginal ultrasonography, we investigated loss/irregularity of the echolucent area between the uterus and the placenta (‘clear space’), thinning or interruption of the hyperechoic interface…

Placenta Accreta Spectrum Disorder in a Patient with Six Previous Caesarean Deliveries: Step by Step Management
2021
The definition placenta accreta spectrum disorders (PAS) introduced by FIGO (International Federation of Gynaecology and Obstetrics) indicates an abnormal, pathological adherence or invasion of the placenta. The growing worldwide incidence of this pathological entity, and the possible serious correlated surgical risks, has caused a significant increase in attention among the scientific community. Previous caesarean delivery and presence of placenta previa are the main risk factors for the onset of PAS. Here, we present the intriguing case of a 39-year-old woman, at the 33rd week of gestation, with six previous caesarean sections and with a diagnosis of placenta previa accreta. At our referr…
